What to Include

  • Description of the returned item(s);
  • Receipt number;
  • Refund method (cash, check, credit card, other);
  • Seller information (name, address, phone, email);
  • Signatures of both parties;
  • Summary of charges:
    • Cost of the item(s) per unit,
    • Quantities of item(s),
    • Subtotal,
    • Tax rate,
    • Total taxes due,
    • Total amount due,
    • Amount refunded, and
    • Outstanding balance (if applicable); and
  • Transaction date.
