A jewelry receipt is the written record of the purchase of a piece of jewelry (such as bracelets, necklaces, and rings). It includes identifying information about the buyer, seller, and jewelry. The jeweler provides this document to the buyer when the transaction is complete. Buyers can use the receipt as proof of purchase when insuring pieces of jewelry.

What to Include

  • Buyer information (name, address, phone, email);
  • Jewelry description (year, color, material, gemstones, condition);
  • Payment method (cash, check, credit card, other);
  • Receipt number;
  • Seller information (name, address, phone, email);
  • Signatures of both parties;
  • Summary of charges:
    • Cost of the item,
    • Cost of add-ons (if applicable),
    • Quantities of items and add-ons,
    • Subtotal,
    • Tax rate,
    • Total taxes due,
    • Total amount due,
    • Amount paid, and
    • Outstanding balance (if applicable); and
  • Transaction date.
