  • Summary of charges:
    • Cost of the item,
    • Cost of add-ons (if applicable),
    • Quantities of items and add-ons,
    • Subtotal,
    • Tax rate,
    • Total taxes due,
    • Total amount due,
    • Amount paid, and
    • Outstanding balance (if applicable); and
  • Transaction date.
